How to Make the Gift

Note: We recommend using certified mail for gifts of the type that follow. The date you mail the certificate is your date of gift for tax purposes.

Using the Certificate
  • Fill in "University of Wisconsin Foundation" on the back of the certificate in the section that reads “...sell, assign and transfer unto _______________.”
  • Do not fill in the section that reads, “hereby irrevocably constitute and appoint _________ (Attorney) to transfer...”
  • Sign the back of the certificate exactly as your name appears on the front.
  • Have your signature guaranteed by a bank officer or stockbroker.
  • Deliver the certificate to the Foundation.
Using a Stock Power Separate from the Certificate
Send the certificate to the UW Foundation in one envelope with nothing completed on the back of the certificate.

In a separate envelope, send a stock power.
  • Fill in "University of Wisconsin Foundation" on the stock power in the section that reads “...sell, assign and transfer unto _______________.”
  • Do not fill in the section that reads, “hereby irrevocably constitute and appoint _________ (Attorney) to transfer...”
  • Fill in the stock power with the name of the stock.
  • Fill in the number of shares you are giving.
  • Sign the stock power exactly as your name appears on the front of the stock certificate and have your signature guaranteed by a bank officer or stockbroker.
